Insure Oklahoma

✔  Insure Oklahoma is a program that will pay up to 60% of an Employee’s Monthly Medical Premium Cost upon Approval.

✔  The Qualifications to be Approved are;

• Must be between the Ages of 19 and 64

• Oklahoma Resident

• Meet Citizenship Guidelines

• Must meet under the Income Guidelines for Household Size

• Pay up to 15% for of the Monthly Premium for Self and Qualified Dependent

• Knowledge of Gross Income

✔  Low Deductible and Out of Pocket Maximum upon Approval.

✔  Insure Oklahoma will Reimburse Money Spent on Copayments and Deductible Amounts Paid.

✔  Insure Oklahoma will cover 100% of Medial Premium if Employee has their CDIB Card.
